Found in Garden City Ward Records: "Phineas H. Cook, b. 28 Jan 1850, SLC Ut.. Blessed Feb. 1850 by P.W. Cook. Baptized 1860. Confirmed 1865. Ordained an Elder 5 Oct. 1867. Died 5 Oct. 1876." (Garden City Ward Record of Members 1889-1894, Manuscript, Original record book Church History Library, LR 3095 7, p. 32.) He was born while the family was living at Canyon Creek in Salt Lake City."The Life and History of Phineas W. Cook," including his journal, published by the Phineas W. Cook Family Organization, #5 N 700E, Brigham City, UT, p. 61. "Phineas Henry was born the 28th of Jany 1850."

MANTI 1850 CENSUS: Sanpete County, Utah Ter. Phineas W. Cook, age 30, born Connecticut (about 1820) lived with wife Ann Eliza age 27. Children in the household were William Duncan age 10 (the child B. Young asked Phineas to take to Utah with him at Winter Quarters), Harriet B. Cook age 6, Augusta Cook age 3 and Phineas Cook age 1.

FISH HAVEN: Called as one of the first directors for the United Order in Fish Haven by Wilford Woodruff June, 18, 1874 (Des. News 23:307) “Organized the Fish Haven branch of the United Order with the following officers: President, Hyrum Rich; V P, Robert Pope, Sec. John W. Edwin Stock; Treasurer John Stock; Directors Henry Howell, Hugh Findlay, Phineas H. Cook.” DEATH: Phineas W. Cook Family Record, handwritten by P.W. Cook, Church History Library MS 6974, p. 3: "Phineas H. Cook accidentally shot himself the 7th of October, 1876 in the following manner. He was on a hay rack with a double barrel shot gun across his (k)nee. His brother Henry rode up on horseback and gave him an apple to cut in two for them. The gun slipped and the hammer struck the wagon hauns and killed him instantly. (difficult to read here) He was age 26 years 8 months 8 days."
