Note | BIRTH Listed Woodruff, Utah on his Salt Lake Temple Card. However, his birth was in Border, Wyoming 17 July 1886, according to family records. Later he was known as Lew Cook. Garden City Ward Records, claimed he had been born in Woodruff (Family History Library film 7,170. ) Lila Eastman's history of her mother recorded the birthplace Border, Wyoming. It was almost certainly Border because of Garden City Ward Membership Records: Sarah T. Bryson Cook and child were received into the ward July 17, 1887. (Garden City Ward Record of Members 1889-1894, Manuscript, Original record book Church History Library, LR 3095 7, p. 158.) LDS CHURCH Baptism: Garden City Ward Records, , Family History Library film 7,170. Also listed on his Salt Lake Temple Card. Endowment: Salt Lake , Family History Library film 471,831. Noted there: "F.R. William Eastman, nephew." DEATH "Oakland Tribune", dated 7 May 1948. 1900 CENSUS: Woodruff, Rich, Utah. Sarah T. Cook is the head of household. Children in the household are William L., age 14 born July 1885 in Utah; Elizabeth J. age 12, born Jan 1888 in Utah; Bertha B. age 10, born Dec. 1889 in Utah; Newel B. age 8, born April 1892 in Utah; and Charlotte B. age 5, born July 1894 in Utah. 1910 CENSUS: Sarah T. Cook, Woodruff, Utah. She is 42 years old. Children are William L., age 24 and single; Bertha age 20 and single; Newell age 15 and single; Charlotte age 16 and single. 1920 CENSUS: Lew Cook, age 33 is married. He is a farmer and manages a general store. His Wife Gertrude W. is also 33, a teacher in the public schools. No children, but a nephew is living with them, Alton Wilkins, age 16, born in Utah. WOODRUFF: Lew Cook went into business, probably at first with his Uncle Henry Howland Cook. In 1898 when Lew was 12 years old, his father was admitted into the mental hospital at Provo. Lew's Uncle Henry had a store on main street in Woodruff and on the 1900 Census Sarah gave her occupation as storekeeper. Eventually Lew took over management of the store and his mother worked there for many years. Lew Bought the house on the same block as the store. His sister Charlotte Dickson used the house all winter for many years because they lived too far out for the children to go to school. The family took care of each other. Henry took care of his sister Sarah's family and Lew helped his sister Charlotte. WILLIAM COOK PROBATE: On January 30, 1948 the heirs of William Cook were named in a quit-claim deed to Frank Jensen, Sec 16, Two 14:5, Rich County Land Records, Vol U p. 399. Heirs are named: (wife) Sarah B. Cook; Lewis Cook and Gertrude; Newell B. Cook and Edna D.; Charlotte and Orlando Dickson; Lila and Marshall V. Eastman, Paul and Kay Porter.
